I think the basic measurement is taken below the breasts so if she has lost some weight that would explain it.

the number is the amount of inches around the ribcage the letter is the size of breast
so if she lost weight she could go down an inch or two

I think the cup size is based on the difference between the measurement around the breasts and the measurement around the ribcage.
So a slim girl whose measurements were 36/30 would have bigger boobs in cup size terms than one who was 44/40.
