I don't know what you mean by "mainstream" exactly, but pee scenes have popped up in non-fetish vids such as "Barely Legal 1 & 2" (Hustler).Pissing ON someone I haven't seen nor read of, at least since the Golden Age went kerplunk (read a review of the original "Femmes de Sade" sometime).
The Porn Biz has certain self-imposed criteria for vids in regard to what they think is likely to possibly garner a prosecution-- a pragmatic "risk/benefit ratio" (First Amendment absolutists would consider this to be a "chilling effect" & eo ipso illegal).Under the "community standards" precept expounded in Roth v.United States (354 US 476), things boil down to whether a jury will convict, which they rarely do( Justice Douglas, bless him, in his dissent in Roth said:"Any test that turns on what is offensive to the community's standards is too loose, too capricious, too destructive of freedom to be squared with the First Amendement....this is...censorship in one of its worst forms.")But the risk of prosecution is still present. Some people push the envlope, such as Seymore Butts & "fisting," or Max Hardcore & his antics. But these things are not on a proscribed list; this would be "prior restraint" in the US & per se unconstitutional.(Child porn is a separate legal subject entirely from obscenity,NB.)
In general I think most US porn is acceptable under R18. Watch out for those Germans tho.
