Design your own using Microsoft Access. That way it conforms to what you want it to do rather than forcing you into somebody elses idea of what you want it to do.

Access can be bought as a stand alone product so you don't have to lay out money for the entire Microsoft Office Suite.
Although the learning curve is steep it's well worth the time imho.
