What do you need that model release form for?
does it have to cover only stills?
video footage?

What's the full deal with release forms?
I created my own model film release form to cover both stills and film. The model signed it, dated it, printed her name and address (well, current address) and a witness signit too.
So i guess i was covered for the shoot.

What is the situation if the model changes address? i guess that does not affect anything, as everyone is allowed to move location.
