Been OOP for a long time, It was produced by Western Visuals which died in the 90's. It may have been released as early as 1986. I believe it is a compilation of loops produced by WV and not a feature film. O'Brian appears on the cover in a corset with some off screen guy grabbing her tits.

I've seen it on ebay from time to time. The cover looks like this:

Don't confuse it with Double PenetrationS 3 which was around the same time.
