Yes the girls on the screen are who you are talking to. The delay is only about 15 seconds so if you keep talking you will see them do your request while you chat.
And as for how far they go that really depends on the model's modelling levels plus the rules of the channel. IE, knicks stay on at all times.

The fine is from ?10,000 - ?250,000 and we have been told that the producers and girls involved in violating these rules will be dismissed without exception. The new rules include:
NO sucking fingers (ie simulating a BJ)
(You can run your finger round your lips but NOT suck)
NO touching your panties on top or underneath the material
NO mouthing swear words (ie "Fuck me" when on camera
NO rubbing the phone between your boobs or legs or pointing it towards your bumhole
