Did Columbo every get a cast wrong ? and not get the murderer ?

Yes and no. There was a case where an old actress murdered her husband but she was suffering from severe dementia and can't remember doing it and wouldn't live long so her butler and best friend took the rap for it. Columbo knew this and let it happen.
